A thickening of tissue around a nerve between the third and fourth toes causing burning pain.

When to See a Doctor

  • Symptoms that do not improve with rest after one week
  • Severe pain or swelling that limits daily activities
  • Visible deformity or unusual changes to the foot
  • Signs of infection: redness, warmth, or discharge

Common Symptoms

  • Burning pain between toes
  • Feeling of pebble in shoe
  • Numbness or tingling in toes
  • Pain worsened by narrow shoes

Treatment Options

We almost always begin with conservative (nonsurgical) care. Surgery is considered only if those measures don’t bring relief. Your plan is personalized after a thorough exam.

Conservative (nonsurgical) care

  • Wide toe-box shoes; avoid high heels and barefoot walking
  • Metatarsal pads and custom orthotics
  • Activity modification and ice
  • Anti-inflammatory medication (NSAIDs)
  • Corticosteroid injections
  • Physical therapy
  • Extracorporeal shockwave therapy

Surgical options (if needed)

  • Neurectomy (removal of the affected nerve)
  • Nerve decompression (releasing surrounding tissue)

Risk Factors

  • Athletic or physically demanding lifestyle
  • Being overweight or obese
  • Poor-fitting footwear
  • Existing medical conditions such as diabetes or arthritis

Prevention Tips

  • Wear properly fitting supportive footwear
  • Warm up and stretch before exercise
  • Maintain a healthy weight to reduce foot pressure
  • See a podiatrist at the first sign of persistent pain
