Plans We Accept

We participate with many insurance plans. Participation can vary by individual plan, so please call us to confirm your specific coverage.

HMO plans may require a referral

If your plan is an HMO, check whether you need a referral from your primary care physician before your visit. Check your member card or call the number on the back to confirm. Call us if you are unsure and we will help you find out.

Some services may require prior authorization from the health plan. Our office can assist with authorization documentation when applicable. Final coverage decisions are made by the health plan.

Common Insurance Questions

Do I need a referral to see a foot and ankle specialist?

Referral requirements are set by your health plan. Some HMO plans require a referral from your primary care doctor before a visit with a foot and ankle specialist. Check your plan or contact our office if you need help.

What should I bring to my first visit?

Bring your insurance card, a valid photo ID, a current medication list, and any useful medical records or previous images such as X-rays. Completing the New Patient forms before the visit can make check-in easier.

How will I know what I owe before my visit?

Our office can review available benefit information and, when possible, provide an estimate of what you may owe. Benefit checks and cost estimates do not guarantee what your health plan will ultimately cover or pay.

Questions about your insurance plan?

Call our office with questions about participating plans, referrals, or what to bring to your visit. Coverage and benefits vary by plan.
